Permanent Non-surgical Rhinoplasty cannot claim that name without merit. The medical grade silicone, like other silicone “implants”, e.g. heart valves, artificial joints, heart pacemakers, internal stents or tubes, are made of the same material, just in solid form. The advent of silicone, in solid and liquid states, is one of the great triumphs of modern biotechnology. Microdroplet injections of minute amounts 1-3/100 of a CC (5 CCs to a teaspoon) into multiple locations is hardly noticed by the body and remains undisturbed. 

This technique of the non-surgical nose job, also known as liquid nose job comprises 25% of Dr. Kotler’s facial cosmetic surgery practice. And the number of patients is growing as more patients disinterested or dissatisfied with temporary fillers, dubbed “a life sentence” seek a permanent and final solution to their dissatisfaction, particularly if they have one or more surgical rhinoplasties.

TEMPORARY NON-SURGICAL RHINOPLASTY

Temporary, absorbable fillers achieved success and fame to plump lips and replace the loss of fullness that occurs in mid-life. Historically, surgeons found it helpful for use in correcting imperfect rhinoplasties and correcting some structural deficiencies as a substitute for a surgical rhinoplasty. But the biggest negative, of course, is the limited longevity of the temporary fillers.
The popular temporary fillers, Restylane®, Juvéderm® and others are not considered permanent because they are destined to be dissolved by the body. Other fillers have some elements destined to thrive longer than four to six months, often lasting up to a year. All these require periodic “refill.” 

Many patients will consider temporary nonsurgical nose jobs and have a change of heart when comforted by surgeons like Dr. Kotler that use computer imaging to showcase results before a permanent non-surgical nose surgery. Over time, temporary nose fillers are a more costly type of non-surgical rhinoplasty because every year the procedure must be performed again.

They are safe, when properly injected and do satisfy patients, particularly initially. But, as time goes on, many patients become disenchanted with the lack of permanency and the cost. “A life sentence” is what one patient described it. “Filler fatigue” is a more common title.

Fillers After Rhinoplasty (Non-surgical revision rhinoplasty)

Divots and depressions may be the hallmarks of a poor surgical rhinoplasty. Non-surgical revision rhinoplasty can be a substitute for a surgical revision rhinoplasty. Fillers after rhinoplasty are very common and Dr. Kotler has worked with hundreds of patients in need of this procedure. 

Most imperfect rhinoplasties are such because “too much was taken out.” Usually bone at the bridge and or cartilage in the lower half of the nose. The injectable filler re-inflates the depressed areas such that the dips, divots, depressions are gone. The injections allow symmetry to again be present. Filler can correct some unsatisfactory features such as the  so-called “but crease,” that vertical, midline cleft between two tip cartilages. It can raise a low bridge, a typical feature in some Asian and African-American noses

Yes. A non-surgical rhinoplasty is non-invasive and involves no incisions. 

Meanwhile, there might be scarring depending on the surgical procedure (open vs closed rhinoplasty).

The majority of Dr. Kotler’s procedures are performed using the closed technique. In closed rhinoplasty, the incision is internal, with no visible scarring.

At the consultation, the surgeon explains the process. An ointment that anesthetizes the skin (an anesthetic or numbing cream) is applied 45 minutes before the skin is numb for the needles that will puncture the skin to place the dermal filler.

Photographs are taken before the injection to document the “before” appearance of the nose. They guide the subsequent treatments and are an excellent part of the patient’s archived medical records. 

After the skin is cleansed with an antiseptic, the injections are made.

It is a cosmetic procedure, but it is not cosmetic surgery. A plastic surgeon is happy to have these injectable products in their toolkit.

Before a non-surgical rhinoplasty, there are only two steps needed. The first is to take medical-grade photographs, and the second is to prepare the nose by applying an anesthetic cream to the skin.

Applying the anesthetic cream 45 minutes before the process makes the skin numb, and the penetration of the skin by a needle and cannula is virtually painless. 

Unlike surgery, which is performed under an anesthetic and requires a pre-op and physical exam, none of that preparation is necessary.

During non-surgical rhinoplasty, dermal filler is injected through a fine needle or cannula underneath the skin and placed between the skin and the bone and cartilage architecture or framework of the nose. 

Side effects are usually non-existent, and there is rarely swelling or bleeding. There is no downtime. One leaves the office sporting the improvement created by the liquid rhinoplasty.

After a non-surgical rhinoplasty, there is nothing to do except leave the office. Sometimes, a little ice is applied to the nose to reduce the minimal chance of swelling or bruising. Otherwise, the procedure has no recovery and no downtime.

Dermal fillers for rhinoplasty are an excellent accompaniment to a surgical rhinoplasty if the results are pretty excellent, but there are some minor points of imperfection after the surgical procedure. 

Rather than have another plastic surgery on the nose, the wise plastic surgeon will consider using a dermal filler to correct the slight to moderate imperfections. 

There is a favorable rate of predictability for a non-surgical rhinoplasty compared to a revision rhinoplasty when dermal fillers are used. The side effects are virtually nil. 

Where the non-surgical nose job employs Restylane® or Juvéderm®, the well-known fillers used in other face parts can be appropriate for the nose. The non-surgical nose job can be an excellent alternative to cosmetic surgery.

A non-surgical rhinoplasty can fix an asymmetry of the exterior of the nose. It can improve a low nasal bridge, conceal a bump, and make a tip look narrower. It can act to give the appearance as though an overdone nose with the absence of cartilage carries a natural appearance. 

There are two types of non-surgical nose jobs, also known as liquid nose jobs. There are temporary fillers composed primarily of a natural substance called hyaluronic acid. The most popular brands are Juvéderm® and Restylane®.

These temporary fillers need to be renewed, typically lasting from four to six months up to one year, though some may last longer, but never permanent.  

The alternative is a permanent non-surgical nose job which lasts forever. In this case, the filler is a medical-grade liquid silicone. This product is marketed under the trade names Silikon 1000 and Adato Sil 5000. 

Medical-grade liquid silicone has carried Food and Drug Administration (FDA) approval longer than any temporary fillers. Its safety is unquestioned. It is done using the microdroplet technique and is indeed permanent. 

After three injections, the result will be achieved, and the patient goes on their way, having concluded the process.

A liquid nose job is considered affordable. It may be only 20% of the surgical rhinoplasty cost.

The results are obvious immediately, and one avoids the uncertainties of surgery and the additional burden of time off work.

If you consider using fillers as “reshaping,” the answer is yes.

Side effects are extraordinarily rare. For example, silicone does not engender infections. 

Typically, there are very few reactions, although some people get a little bit of redness, which is attributable more to the injection than the deposit of the Silikon 1000 filler itself.

There is little risk of complications because of the tiny needle used and the small amount of filler deposited under the skin. The risks are more related to the administration. 

The surgeon must be comfortable with the nose anatomy and ensure he is positioned away from the major blood vessels.  

That anatomy is consistent, so confident practitioners must “know their way around.”

Using a very fine needle and drawing back on the plunger before injecting gives assurance that the needle tip has not entered a vein or artery. 

That is the safety measure that translates to less chance of any complications. Infections are virtually unheard of, and there is minimal reaction to the product.

There are so-so, good, and great filler injectors. In the case of ham-handed injectors, fillers could result in side effects like lumps, infections, and discolorations.

It matters who does the injections.

There is no recovery. Office treatment from dermal fillers, whether temporary or permanent, whether one or more areas are treated, are the same. One leaves the office, and the process is over.

With a liquid nose job, there is no downtime. There are no side effects. Of course, it is a surgical procedure with anesthesia, recovery, and the need for some elements of home care for the first couple of days.

The non-surgical rhinoplasty is not uncomfortable because an anesthetic ointment is placed onto the skin 30 to 45 minutes before the appointed injection time. The patients find this very satisfactory and helpful. 

When injected with Restylane®, Juvéderm®, or medical-grade liquid silicone, the permanent liquid rhinoplasty vehicle, the patients will feel very little pain or discomfort. 

Some people feel it is unnecessary to have local anesthesia for dental work and prefer not to have topical anesthetic. They are satisfied, comfortable, and enthusiastic about the procedure without the topical anesthetic.

Most patients consider a non-surgical nose job or liquid rhinoplasty well worth it. The charges are far less than those of surgery. However, the temporary fillers over time may be a constant expense that could even exceed the cost of surgery.

The cost difference is where the permanent non-surgical nose job carries the banner. This option does not need renewal because the body accepts and maintains the filler. There is no degradation of the medical-grade liquid silicone dermal filler. 

Hence, a non-surgical nose job is an excellent alternative to plastic surgery, in particular, because of its longevity and fixed cost. It also has no side effects or downtime. 

A non-surgical nose job is a convenient cosmetic procedure because it allows fine-tuning with minor corrections, which may still plague a patient after prior plastic surgery on the nose.

Non-surgical rhinoplasty is indeed non-surgical. It is done in an office setting and requires little preparation. There is no recovery time. 

Surgical rhinoplasty typically can address many more problems, including breathing problems, but it is an operation. 

It is conducted in an outpatient surgery center under anesthesia. There is a seven- to ten-day recovery period. The costs are significantly different. 

A desired result from a surgical rhinoplasty cannot be warranted. Therefore, some patients must have a revision rhinoplasty.  

At that point, the question should arise as to whether the revision should be a temporary non-surgical revision rhinoplasty or a permanent non-surgical revision rhinoplasty.

The advantage of this option is you avoid surgery. There is no downtime. There are no side effects. The cost is far less. There is greater precision of treatment. You avoid anesthesia.

  Non-surgical rhinoplasty is more likely to produce satisfaction for certain imperfections than surgery, which always carries more uncertainty. 

No procedure is ideal for all patients. Since the non-surgical rhinoplasty procedure is a dermal filler, it is an “addition” to the nose. 

When a plastic surgeon performs a traditional rhinoplasty, it is generally a “subtraction” process. Some bone and or cartilage is removed. 

For one with a large nose, while capable of obscuring minor imperfections, the liquid rhinoplasty will yield a net increase in nose volume, which may not be appropriate.

The traditional rhinoplasty is very effective as a subtractor of excess bulk and volume of the nose.

A huge nose would likely not be well served by a non-surgical nose job or dermal filler. 

Increasing the volume of the nose is likely to be less satisfactory. So, in that case, the traditional surgical rhinoplasty is appropriate since it is a “subtraction” procedure.

Traditional rhinoplasty carries some uncertainty, side effects, and increased downtime, which are difficult for some people to swallow, particularly if they have had prior surgery.

 If you are unhappy with the appearance of your nose or breathing, you need to see a specialist. Family doctors and internists are of little help because they are not surgeons. 

The cosmetic nasal surgeon who works in nearly or exclusively rhinoplasty should be at the top of your list of consultants. 

The “superspecialist” generally has a computer imaging system to show you the predicted result. After all, shouldn’t you see the expected outcome?

The best candidates for non-surgical rhinoplasty are those who have imperfections after a surgical rhinoplasty. 

Those that match the normal components of the nose will be successful. These facial features include grooves, troughs, dips, pinches, asymmetries, divots, and abnormally low bridges. 

Raising the space between the nasal tip and the dorsal hump puts this dermal filler into an area where the nose is too thin on one side. 

Other good candidates for liquid rhinoplasty are those born with a low nasal bridge. Asian patients typically have that. 

One raises the bridge by placing dermal fillers under the skin to achieve a more Westernized nose look. 

Likewise, there may be a rounded or slightly depressed tip. Therefore, placing the filler underneath the skin at the tip will give the appearance of more cartilage and, thus, a more projecting and satisfactory tip.

A patient is not a good candidate for injectable fillers if their nose is very large. After all, the permanent non-surgical rhinoplasty adds volume. It is a filler. It is best for a very lean, thin, and obviously imperfect nose. 

Those who have a large nose, to begin with, will not be satisfied when the permanent filler increases the volume of the nose. 

Liquid rhinoplasty, which uses dermal fillers, is considered very safe. In the hands of a highly specialized surgeon with long experience, it is a procedure that is extraordinarily safe. 

As the plastic surgeon knows anatomy, it is not likely to interfere with the course of any blood vessels or nerves. 

There is very little chance of any breakdown of the skin. Infection is extremely rare. There are no other significant side effects. It has a long track record of success and a low incidence of complications or problems.

Most non-surgical rhinoplasties are temporary due to the fact that almost all the practitioners have been trained and educated only in the use of absorbable fillers. 

Restylane® and Juvderm are the best-known injectable dermal fillers, albeit temporary. While there is variety, they all have a timeline and ultimately will disappear. 

Only one product has professional recognition and FDA approval to be described as “permanent.” 

The only permanent non-surgical rhinoplasty is the use of medical-grade liquid silicone to correct those bumps, divots, irregularities, and asymmetries. It is excellent for correcting a pinched tip and an over-sloped nose. Indeed, it is permanent.

Most plastic surgeons administer temporary hyaluronic acid filler for non-surgical rhinoplasty procedures. 

Although these options can be suitable, they yield only temporary results and require regular follow-up injections.

Rather than using temporary fillers, Dr. Kotler offers his patients permanent solutions.

Non-surgical nose jobs that are done using the permanent filler will last forever. Consequently, patients opt for a permanent nose filler that does not have to be “refilled.”

Whether it be to hide a dorsal hump, raise the nasal tip, or correct the appearance of a crooked nose, the injections are permanent and therefore mimic the results of nose surgery.

Silikon 1000 (purified polydimethylsiloxane) is a trade name for clear, colorless, medical-grade liquid silicone. 

The product is manufactured for use as a permanent filler in appropriate areas. It has been used for acne scars with some success. It lasts longer than any of the collagen injectables. 

Silikon 1000 is FDA-approved and has over a 60-year track record of success. The original use for the product was to “push back” a detached retina. Generally, there are very few issues with the product when properly used.

 It should be noted that the most satisfied patients had the permanent filler, not the temporary filler, because the latter implies a lifetime of treatments. 

Conversely, the permanent filler treatments are finite and end when a limited number of sessions are completed. Therefore, there is no recurring and constant cost of renewing the temporary dermal filler.

One cannot use Silikon 1000 in conjunction with temporary fillers.  

The reason is that temporary fillers do not provide an adequate environment for the injectable silicone oil to become embedded and surrounded by nature’s self-generating capsule. 

It will settle best and remain constant when injected into tissue that has had no alteration by the injection of temporary filler. 

There is no benefit to combining a temporary filler with a permanent filler. If one prefers temporary fillers, which some deem a “life sentence,” that is one’s prerogative. Most patients will not relish that consideration. 

Popular temporary dermal fillers include Restylane® and Juvéderm®.

Sometimes it can, and sometimes it cannot. By itself, it certainly cannot correct breathing issues. It cannot fix some major asymmetries and specific congenital abnormalities. 

We have to pick and choose which modality is better, surgical or non-surgical, and then if non-surgical, permanent versus temporary.

In reality, the non-surgical nose job or liquid rhinoplasty is additive. It adds volume. However, it is appropriate in many circumstances where too much of the nasal architecture has been removed during an unsuccessful plastic surgery on the nose. 

When cartilage or bone removal has been excessive, there are only two options to correct the problem. 

One is another surgical rhinoplasty where bone or cartilage is grafted from the rib cage, the ear, or the nose to the space between the skin of the nose and the existing deficient skeleton or correction by the dermal fillers.  

The latter are simpler, more predictable, carry virtually no side effects, and have no downtime. 

Accuracy is the other calling card of a non-surgical nose job because the filler volume used to level out the irregularities and asymmetries, dips, divots, and potholes is minute. 

The doctor delivers the filler through a small needle or cannula. This treatment plan should be the prime consideration when the patient presents to a facial plastic surgeon with an unsatisfactory nose job.

A non-surgical nose job cannot structurally narrow the nose. 

Still, it can create the illusion of narrowness because the filler is placed in the midline of the nose, usually to obscure a dorsal hump or an asymmetry or some of the dips and divots in the midline. It has the same visual benefit as narrowing the nose. 

The best use of a filler to give the appearance of a narrow nose is in patients with a low bridge. When the bridge of the nose is filled, they will not have a dorsal hump. 

This feature is very common for Asian patients who have a low bridge, a likely wide bridge, and sometimes a slightly wide tip.

But by raising the bridge, you will have the illusion that the nose is narrower. That presentation lends itself well to filler, and, ultimately, the nose will look narrower.

Treatment should always be performed by the most experienced, most specialized practitioners. There is no substitute for experience, and there is no substitute for narrow focus. 

Injection of Silikon 1000 is an art form that rests in the hands of a skilled practitioner. It is challenging to learn because it yields a permanent change. One learns best from an experienced preceptor or trainer. 

Dr. Kotler was fortunate to have that experience very early in his career. He observed that this technique would be a marvelous adjunct to a cosmetic nasal surgery practice.
